Empowering Choices: The Life-Changing Impact of Person-Centered Options Counseling

Understanding the options of long-term care and health services can be overwhelming for seniors and their caregivers. Through the MI Options program, CareWell Services Southwest offers Person-Centered Options Counseling (PCOC) to help individuals understand their options and find the resources they need to age in their homes.

The goal of PCOC is to ensure that long-term care and health options align with an individual’s personal goals and circumstances.

“[MI Options PCOC] helps explain the long-term services and support available within our community for those who are no longer able to live fully independently,” said Kim Smallcombe, Community Living Manager at CareWell Services “Many people think the only option is a nursing home when they need assistance with their daily living. That is not the case.”

This individualized focus is the cornerstone of the program. Rather than offering one-size-fits-all advice, counselors work to tailor plans to each person.

“Every person’s life experiences and health conditions are unique. There is no cookie-cutter solution that works for everyone,” said Smallcombe. “We employ the use of person-centered counseling to listen deeply to their needs, wishes and concerns, providing information for all of the various options, so that they can select what best matches their vision for today and into the future.”

This specialized support was crucial in Branch County for the wife of a man with dementia whose active Power of Attorney was suddenly revoked. Facing a situation where her husband was potentially unsafe to himself and others, she received help from a PCOC counselor who assisted her in securing temporary guardianship and even attended court with her. With the continued support of the PCOC counselor they were able to navigate a complex web of doctors and get him the care he needed.

“PCOC is a great tool, whether you need services now, or want to plan for the future,” said Smallcombe. “It’s also especially helpful for those who may be worrying about their aging parents, or who have been suddenly thrust into a caregiver’s role. We help answer all of the questions so you can proceed with confidence.”
