Eligibility Requirements
Eligibility screening services are available at CareWell Services’ offices at the Kool Family Center in Battle Creek.
Individuals are asked to provide the following items at the screening:
Proof of age and Calhoun County residency with Driver’s License or State ID card
Proof of income by showing check stub, tax return, bank statements, Social Security income statement
Copies of all insurance cards, such as Medicare, Medicaid, VA, Part D, Medigap, Medicare Advantage or others
Battle Creek Hearing and America’s Best Vision are partnering as the providers for services.
