As part of the MI Choice Medicaid Waiver program, CareWell Services Southwest maintains a pool of direct service purchase vendors within our service area to provide quality service to our participants. If a participant’s Plan of Care justifies services, the Supports Coordinators will contact a provider for the provider pool to request services. If you would like to become a provider with CareWell Services Southwest, you will first need to ensure you meet the following requirements:

  • Complete the Application Packet
  • Meet all program requirements as defined in the Operating Standards
  • Maintain local, state, and federal requirements

Kinship Request for Proposals

CareWell Services Southwest is accepting Request for Proposals (RFP) applications from organizations interested in applying for a grant with an award date of March 1, 2024 through September 30, 2025 for kinship services in Calhoun County. 

Eligible applicants include for profit organizations, nonprofit agencies, and units of government.

Calhoun County 911

UPDATE: AT&T wireless is currently experiencing a nationwide outage. You may have difficulty calling 911 if your cell carrier is AT&T or your provider uses the AT&T wireless network.
 
For affected customers: If you need to get ahold of 911, first attempt to use your cellular device as 911 may still work in ‘SOS’ or similar emergency mode.
 
If you are unable to get through, try calling while connected to WiFi with settings that allow calls on WiFi mode. It may take a couple minutes for your call to go through.
AT&T is working to correct this issue.
