Immunization forms will need to be completed no later than November 14 in order for them to be billed to your insurance before the clinic. Insurance Forms will be available at the FSC Front Desk or Family Fare Pharmacy. If you would like to know what vaccines you may be due for, call Family Fare Pharmacy at 517-629-2286. The pharmacy is open 9am to 7pm Monday through Friday and 9am to 5pm on Saturday. Don’t forget to also sign up on the appointment schedule on the counter in the Social Room.
