Providing proofs for your programs––Look for the purple envelope!

I can’t believe it’s already the end of 2023! With the new year fast approaching, it’s the time of year where programs are verifying continued eligibility by asking for proofs. Proofs are copies of documents which show your residency, age, sources and amounts of income, plus insurance card information. 

These documents are used to determine starting and ongoing eligibility for grant or taxpayer-funded programs. Subsidized housing, food support programs, Medicaid insurance, and County Millage services like Meals on Wheels and Personal Emergency Response buttons are just a few of the programs which might require these proofs. 

Social Security income and Medicare insurances often change at the start of each year. You will get letters with your income and insurance details. Please save and set aside these proofs, along with any pension or investment income statements, so they are available when requested for eligibility redeterminations.

CareWell Services will begin annual redeterminations in January, 2024 for its non-Waiver clients using Personal Emergency Response Service (PERS) buttons. If you use a PERS button from CareWell, you may receive a light purple envelope with the necessary information for sending your proofs. Keep an eye out for the light purple envelope starting next month!
