CareWell Services Southwest has specially trained and certified resource specialists available from 8:00 am to 4:30 pm, Monday through Friday, to answer questions and provide information on local services for older adults, persons with disabilities, and family caregivers.

Our certified Resource Navigators are available to provide information and assistance on thousands of services and resources available for older adults (60+), persons with disabilities and family caregivers in Barry and Calhoun counties. We can even connect you to agencies and services in other areas of Michigan if needed. CareWell Services is also the MI Choice Waiver agent for residents of Branch, Barry, Berrien, Cass, Calhoun, Kalamazoo, St. Joseph, and Van Buren Counties, and can assist people who meet certain income and health requirements remain in their homes.

Areas of assistance include, but are not limited to, housing, eligibility and availability of government-funded long term care programs, transportation, Medicaid/Medicare assistance, personal care, respite care, and referrals for prescription assistance. Depending on the information requested, a listing of the various services and resources, tip sheets and other materials verified and trusted from our resource directory will be mailed to the caller.

To best serve callers, the Resource Navigator may need to have information on income and health status of the older adult or person with disability. CareWell Services Southwest is HIPAA compliant and all information provided is confidential.
